Job summary

We have an exciting opportunity within the integrated discharge team for a band 7 Discharge nurse. The successful candidate will work alongside a team of nurses and take a lead in managing complex patient cases as an autonomous practitioner. You will be expected to lead a team of nurse alongside our other established band 7 nurses. Experience of discharge planning skills and carrying out patient assessments to facilitate safe discharges of all patients who sit under the discharge team process is an essential. The post will involve attending wards daily educating junior members of the team along with educating ward teams and all members of the multidisciplinary teams. The successful candidate will work closely with all members of the wider system to ensure a Home First approach is followed from the moment the patient is admitted to hospital. Main duties of the job

The successful candidate will spend most of the working day co coordinating safe discharges. Carrying out meetings to establish capacity and support the Discharge Nurses on the wards with any complexed issues

To work collaboratively, as part of a Trust wide IDT, with ward based multi-disciplinary teams and community health and social care partners to agree discharge plans that reflect the ongoing care needs of patients, and respect their wishes

To enable patients to leave hospital the day that they are identified as being community ready, in order to minimise discharge delays, improve inpatient flow and reduce the possibility of clinical deconditioning

Work with the ward clinical team to ensure safe and timely transfer of patients to the Discharge Lounge once a patient has been identified as community ready on the Red to Green Launchpad.

You will be act an educator and senior support within the IDT and the wider system.

The Ward/Department Manager has 24 hour accountability for patient safety and patient experience on their ward/department, including line management of their staff.

They must deliver high quality nursing care by ensuring appropriate systems and processes are in place to provide the continuity of safe and effective care at all times.

They will act as a champion for professional accountability and standards, ensuring that professional practices are in line with the requirements of the Nursing and Midwifery Council (NMC), and Trust Policy.

Ensure environmental standards and cleanliness of the ward/department are maintained in line with Trust and National standards.

Effectively manage the ward/department nursing resource, ensuring that staff rotas are able to deliver safe and effective patient care.

Support and contribute to the development of patient care and nursing practice within the Directorate. Supervising junior staff. Contribute to Trust wide projects as required.

Ensure effective line management of staff and ensure they are supported in performing to their full potential. Ensure junior staff have appropriate support and supervision and are appropriately assessed.

To enable patients to leave hospital the day that they are identified as being Community Ready, in order to minimise discharge delays, improve inpatient flow and reduce the possibility of clinical deconditioning

To be an active role model on the wards areas, to ensure safe and timely transfer of patients to the Discharge Lounge once a patient has been identified as Community Ready on the Red to Green Launchpad

To coordinate the service on a daily basis ensuring all patients are considered for Home first using the Discharge to Assess Model.

To be responsible for coordinating services that enables the patient to be discharged within two hours from their arrival into the Discharge Lounge to the ongoing transfer of care to our community and/or social care teams

To Ensure accurate data is collected and submission of all sit-rep data for example weekly delay transfer of care and long length of stay reviews.

Active monitoring of key Discharge to Assess metrics including Right to Reside coding at ward level and emergency readmissions within 30 days to support continuous service improvements of the discharge pathways

Team work across many professional, community services, adult social care and the voluntary sector providers

To work as the trusted assessor to undertake patient discharge assessments on behalf of other partners including care homes, community health and social care in accordance with agreed protocols/parameters

Will manage the IDT staff and coordinate the daily case load supporting team mebers of the team working within the ward areas.

Effectively line manage a team within the department, ensuring that staff are supported, supervised and have appropriate access to ensure they are able to meet all mandatory compliance metrics.

Must lead by example and motivate and empower others within the Integrated Discharge Team

Must possess the specialist skills and expertise to provide advice and guidance on all aspects of simple and complex discharge and ensure the highest standards of patient care are delivery.

Will manage the roster ensuring that the service remains within budget, safe, fair and effective over 7 days between 08:00 - 20:00.

Will act as a champion for professional accountability and standards, ensuring that professional practices are in line with the requirements of the Nursing and Midwifery Council (NMC) and/or the Health and Care Professional Council (HCPC) and Trust Policy

Liaise with ward staff, members of the multi-disciplinary team, patients, relatives, carers, primary care and social work department, both hospital based and community area teams

Escalate any delays appropriately in real time to Matron or relevant Directorate Manager for resolution to achieve same-day discharge.

Attend where appropriate, multi-disciplinary meetings, case conferences and best interests meetings to support the setting of goals for patients with complex discharge planning needs at the beginning of their hospital stay.

Attend multi-agency meetings as deemed necessary and feedback to line manager and the wider team

Represent IDT at various relevant operational/quality improvement meetings and feedback as necessary to Matron and Directorate Manager
