Job summary

We are seeking to expand our health improvement team within Community Services across South Tyneside and Sunderland FT. We are looking for dynamic and innovative individuals who is as passionate about community services as we are, to join our team.

You should be a professional who is an excellent communicator with experience of working in an outreach role in the community. You will be visible and accessible with support from the Community Specialist Health Improvement Practitioner to help deliver health improvement programmes across community services.

Main duties of the job

The post holder will:-

Support underserved/hard to reach groups to access services.

Work across a range of geographic locations in the community.

Be able to engage and communicate with a range of service users, general public, staff and other agencies.

Support innovation, service improvement and service user experience initiatives.

Work with full age range including children and young people.

Contributes to and advises on the development and implementation of resources for specialist local health improvement programmes in Sexual Health, Mental Health and Community Services.

Maintains public health information such as Equality Impact Assessments and Chlamydia Screening Programme.

To provide and deliver education and training programmes in community, workplace, partner organisations and educational settings.

The post holder will be responsible for supporting delivery of resources for Chlamydia Screening Programme in accordance with National Chlamydia Screening Programme (NCSP) and British Association for Sexual Health and HIV (BASHH) standards.

To deliver outreach programmes in Sexual, Mental Health and Community Services.

To support promotion of sexual health, mental health and community services through marketing campaigns, digital media and links/network with other agencies/services.

Supports patient and service user engagement, experience, inclusion and involvement including co-production especially with utilisation of resources.
