We are looking to recruit a highly motivated
and enthusiastic Specialist Palliative Care Nurse to work within the hospital
team in Sunderland. You will be joining
an experienced team of Specialist Palliative Care Nurses, and will be part of
an integrated multi-disciplinary team who work from the purpose built St
Benedict’s Hospice and Centre for Specialist Palliative Care
The post holder will be responsible for
providing the highest standard of expert holistic care for patients and their
families managing their own caseload and providing specialist palliative care
nursing, advice and support to patients, carers and professionals across
Primary and Secondary Care.
Provision of education to colleagues across
the health and social care sector, both formal and informal, is an integral
part of the role.
Main duties of the job
The applicants should have considerable,
recent, experience of providing palliative and or cancer care to patients in a
senior nursing role at Band 6 or above in either the community or hospital
environment.
You should be/have:
• RGN
• Teaching
Qualification ENB998/ or equivalent
• Relevant
Degree
• Palliative/Cancer
Care qualification e.g. ENB 931/285/237
• Clinical
skills qualification or be willing to undertake relevant training
• Independent
nurse prescribing qualification or be willing to undertake relevant training
• Excellent
communication and leadership skills

A specialist palliative care nurse is defined as a qualified nurse whose primary role is to provide care, guidance and support which is based on an advanced level of knowledge to patients, carers and health professionals concerning a specific disease/condition.
The specialist palliative care nurse will:
Continue to develop advanced knowledge and skills within specialist sphere of practice.
Manage a clinical caseload, reviewing patients and play a key role in optimising complex treatment regimes within specialist palliative care, facilitating complex hospital discharge and liaison with multi- professionals to improve the patient journey by increasing access to on-going assessment and appropriate care and treatment post-discharge.

Continuously improve clinical standards in line with clinical governance framework.

Provide direct support for palliative patients, either face to face or telephone consultation.
Represent specialist palliative care regionally and nationally.
Play a key role in ensuring that national and local standards/targets for the specialist area are met.
Undertake research relevant to the needs of specialist palliative care practice.
