This is an opportunity for an
outstanding individual to work with us in community nursing as a Junior Nurse
Practitioner. You will be part of a
multi-disciplinary community integrated team and you will be part of the
clinical leadership team of community nurses working collaboratively to
continually develop the integrated teams objectives.
You will work in partnership with
District Nurses, GP Practices, Local Authority, Acute services, and other
professionals across Sunderland .
You will be supported by, and will
provide support to the Band 7 Community Nurse Practitioner
The aim of the role is to
proactively manage high risk patients with complex long term conditions,
including frailty and deliver high quality care in partnership with the
patient, carer and other agencies in order to maintain independence, maximise
quality of life, reduce the risk of avoidable hospital admission, and enable
patients to die with dignity in their preferred place of care. You will undertake comprehensive assessment
of health and well-being, recognising the early symptoms of disease
exacerbation, identifying any anticipated emergencies and completing current
and future plans of care based upon the understanding of the disease process,
current evidence and practice standards.
Main duties of the job
The post will be within one of the
Community Integrated Teams in Sunderland, and you will be required to work
across Sunderland as required.

Duties are across Monday to Friday
8:30-17:00 however this may be subject to change. You will be based with one of our Community
Integrated Teams in Sunderland
You should be an RGN with:
- Current
NMC Registration (Level 1 adult nurse)
- Degree
in nursing studies or healthcare related subject or evidence of working towards
- Mentorship
qualification/equivalent and or Practice Assessor training.
- Evidence
of CPD
- Clinical skills accreditation or
willingness to complete
- Non-medical prescribing or
willingness to complete
- 2
years minimum experience as a Band 5
- Thorough
understanding of contemporary nursing issues
- Experience
of leading change management in ward/dept area
- Evidence
of innovation in practice
You
must be able to meet the travel
requirements of the role i.e. have a Uk drivers licence and have access to a
car
