The Day Hospital is an award winningservice that uses an extended scope of practise framework for holistic healthcare management. The service specialises in management of falls, stroke rehabilitation andParkinson's disease as well health preventionstrategiesandtreats a wide range ofmultipathologies affecting the older person.We aim to support individuals to maintain independence and manage their long term health conditions effectively, reducing hospital admissions and helping to facilitate early supported discharge of patients from hospital with our outreach service, ambulatory care team and collaboration with the acute older persons Hospital at Home service
Main duties of the job
This service offers a range of interventions in both an outpatient setting and within the patient's home environment. As a band 6 nurse you will play a vital role in the delivery of high quality patient centred care to older people with a variety of complex health and social care needs. You will work collaboratively within a multidisciplinary team. This role offers opportunity to develop skills in holistic assessment, care planning and the delivery of a range of treatments and interventions across different settings across Christchurch and Poole Hospitals and within the community.
A full drivers licence and access to a car for work purposes is essential for the role.
This role requires an active UK NMC registration
