Main duties of the job

This position plays a pivotal role in Trust's commitment to medication safety, collaborating with all disciplines to foster a culture of continuous improvement and safeguard patient well-being.

Incident Management

  • Review, investigate reported incidents, and identify and analyse risks reported via the Trust's incident reporting system (Datix).
  • Facilitate and lead System Analysis (SAIs) and other investigative techniques for significant medication incidents, developing robust action plans to prevent recurrence. Work in line of the Trust Patient Safety Incident Response Framework.
  • Identify and report trends, themes, and learnings from medication incidents to relevant committees, departments, and staff.
  • Monitor the effectiveness of implemented safety initiatives and make recommendations for further enhancements.
  • Raise awareness of medication safety issues and promote a proactive safety culture through presentations, workshops, and communication campaigns.
  • Work closely with multidisciplinary teams to champion medication safety initiatives.
  • Foster a culture of open reporting, learning, and continuous improvement regarding medication safety.

Policy & Procedure

  • Develop, review, and update Trust-wide medication policies, procedures, and guidelines in line with national recommendations, best practice, and regulatory requirements (NICE, MHRA, CQC).
  • Ensure that all medication-related policies are accessible, understood, and implemented consistently across the Trust.
