Job summary

Sandbanks ward is a 23 bedded ward specialising in oncology. Our patients include those experiencing side effects from their chemotherapy and radiotherapy treatment, patients requiring brachytherapy, ablation and a who are needing treatment for symptom control. Due to the nature of our patients condition we also work closely with hospital palliative care and the end of life team.

A discharge coordinator's main duties revolve around facilitating smooth and safe patient discharges from hospital. This includes coordinating all aspects of the discharge process, from initial planning to the patient's departure, and ensuring effective communication and collaboration between the various members of the MDT. They work closely with patients, families, healthcare professionals, and external agencies to ensure a positive and successful discharge experience.

Main duties of the job

To work on Sandbanks as a Clinical Discharge Coordinator to achieve effective discharge practice in the management of complex cases.

- Works as a clinically independent and autonomous practitioner supporting the multi-disciplinary team to facilitate safe, effective patient discharge as an integral part of the ward team/ integrated discharge co-ordination team

-To be a skilled and knowledgeable advisor for patients, family/carers and staff, raising understanding of the Trust's Discharge Policy, Discharge Planning Standard Operational Procedures and discharge planning processes for simple and complex discharges.- To act in the capacity of a Discharge expert within the trust. Advise and actively support Sandbanks ward whilst liaising with the patient, family, ward MDT, Single point of Access (SPA) to progress timely discharge from the acute trust.

Any other changes not related to this relocation will be managed according to the relevant Trust or National terms and conditions.
