Job summary

An exciting opportunity has arisen for a knowledgeable, motivated Discharge Keyworker to join the Stroke team at Bournemouth Hospital.

This post offers the successful individual the opportunity to enhance their discharge knowledge and practically support both patients and staff ensuring that patients experience a safe discharge.The successful individual will spend the majority of their working day with the MDT and liaising with patients, relatives and the wider discharge network to streamline complex discharges:

You would be expected to:o Support the ward MDT with assessment and decision-making.o Liaise between the ward staff, therapists, social workers and external partner's staff.o Guide ward staff to improve discharge planning referrals/ process.o Support effective discharge planning reporting via the discharge related IT systems.

o Complete appropriate referrals and discharge paperwork

o Booking joint therapy and family meetings.

Base Location: Royal Bournemouth Hospital
