An exciting opportunity has arisen at University Hospitals Dorset to appoint a Macmillan Personalised Care and Support Lead to guide Cancer Services, improve clinical outcomes and patient experience!
This role will allow the successful individual a fantastic opportunity to shape, deliver and embed long-term sustained improvements to advance the services we offer to our patients, and to play a key role in the on-going cancer transformation of this Trust.
The NHS Long Term Plan for Cancer states that "where appropriate every person diagnosed with cancer will have access to personalised care, including a needs assessment, a care plan and health and wellbeing information and support". A 'one-size-fits-all' health and care system simply cannot meet the increasing complexity of people's needs and expectations. Personalised care is based on 'what matters' to people and their individual strengths and needs.

Main duties of the job
The Post holder will coordinate, lead and be jointly accountable for the implementation of a programme of work to deliver the National Cancer Personalised Care Interventions and Stratified Pathway's. This includes the Implementation of Treatment Summaries and Stratified Follow Up pathways in Breast, Colorectal and Prostate. Stratified follow up pathways will be 'rolled' out across other tumour groups over the next 12 months. This programme includes working in partnership with health care professionals across Dorset to develop best practice guidelines.
This post requires a well organised, health or social care professional with experience in project management and/or quality improvement initiatives.
It is a great opportunity for someone with a good understanding of cancer services, experience with quality improvement and project management skills to bring together the Personalised Care and Support (PCS) Programme across University Hospitals Dorset.The role will include scoping and mapping of the existing cancer workforce's capability to deliver the Macmillan's Right by You Principles and be responsible for ensuring their understanding of this as a way of working; also engaging with and influencing healthcare professionals, with a project management function to ensure the smooth delivery of the Personalised care agenda in the Trust.
