Transfer of Care Hub (TOC) Clinician

Job summary
This is an exciting opportunity to be able to contribute and shape the development of a Transfer of Care Hub within acute hospital sites across Dorset. You will provide a consistent presence within the acute hospitals for the Transfer of Care hub. You will work closely with the Cluster leads, acute hospital teams, social workers and community providers to ensure patients have a smooth transition onto the correct pathway contributing towards discharge and flow across the system.
This role is currently fixed term until 31st March 2026 with the intention that this period is used to evaluate and inform the transition to and adoption of a new Transfer of Care Hub model.
Those with current NHS service will be offered on a secondment basis only. Please obtain permission to be released on a secondment before applying.
Main duties of the job
You will be the consistent presence at Poole or Bournemouth hospital on behalf of Dorset Healthcare.
You will be responsible for clinically triaging referrals sent to single point of access (SPA) within Dorset Healthcare Trust and liaising with all teams involved in planning discharges and agreeing patient pathways.
Your role will involve liaising with patients and families on the ward when needed to expedite discharges and support MTD's as part of complex discharge planning
This is an exciting opportunity for someone who is keen to build supportive and strong relationships across partners and in turn make a difference to patient pathways across Dorset.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
