Job summary

Are you a qualified Nurse? Looking for something within the Community Setting? We are excited to announce we are looking for a part time B5 Nurse to support the inpatient rehabilitation team at Coastal Lodge.

Coastal Lodge provides rehabilitation and reablement to enable earlier hospital discharges, support individuals' rehabilitation and prevent hospital admissions. The caseloads provides a wide variety of medical conditions and illnesses including orthopaedic, neurology, respiratory, complexity due to frailty and older person care. Coastal Lodge work Monday-Friday 8-4pm.

The post holder would support the nursing needs of patients under the inpatient rehab setting and would have full access/support from the CBIC and Medical Team. There would be an expectations alongside your nursing skills to be able to undertake generic assessment, develop and implement treatment plans and contribute towards the MDT.

Main duties of the job

The role will involve patient assessment, goal setting, implementation of evidenced based treatment and management strategies, and autonomously managing your own caseload in rehabilitation. Primarily the successful candidate will work towards discharging the patients from inpatient rehabilitation setting through to home-based rehabilitation.

We offer employment in a dynamic team, of Rehabilitation Assistants, Physiotherapists and Occupational Therapists who work as part of a wider MDT.

The successful candidate will gain knowledge and experience alongside a range of colleagues, within a mutually supportive team. They will benefit from support through supervision and annual Professional Development Reviews, and contribute to the training and supervision of rehabilitation assistants.

You will be required to carry a clinical caseload of patients within Coastal Lodge, You will be responsible for ensuring that patients benefit from high quality, evidence-based interventions, which you will deliver and record according to professional standards and legal requirements.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
