Job summary

An exciting opportunity has arisen for an Enhanced Clinical Practitioner (ACP) to join the Jurassic Coast Integrated Community Team/Hub based at Bridport Community Hospital. This challenging role offers the post holder the opportunity to provide high quality patient centred care to frail, older people in West Dorset.

The successful applicant will work together with primary care, secondary care, community services and social services to provide enhanced, pro-active and reactive care as part of an integrated, multidisciplinary team.

We are especially keen to recruit a practitioner with a varied professional background who has the ability to provide advanced clinical assessments and personalised care plans for older patients with complex health and social care needs.

Main duties of the job

Successful applicants must have a broad experience in community and/or urgent care at a senior level. They will be motivated, enthusiastic and have proven experience of working as an autonomous practitioner

Ideally applicants will have the following qualifications:

  • Current registration with NMC/HCPC
  • Relevant professional health degree
  • Evidence of recent CPD, demonstrating alignment to Health Education England multi-professional framework for advanced clinical practice.
  • NMP
  • Post Graduate Diploma in Advanced Practice or specialist practice

This is a part-time post of up to 22.5 hours per week. The service will cover 8:30 to 18:30 Monday to Friday and will develop and be expanded over time including weekends in the future.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Knowledge, Skills and Training

Essential

  • Registered practitioner to degree/diploma level supplemented by minimum PG dip (120 credits) plus MA/MSc or working towards in a relevant subject or demonstrable extensive experience in the relevant specialty
  • Learning and Assessing in Practice Qualification or equivalent practice assessors training
  • Non-Medical Prescriber (if professionally appropriate)

Job Specific Experience

  • Experience at Registered Practitioner Band 6 level
  • Experience of developing specialist programmes of care for an individual or groups of patients/clients and of providing highly specialist advice
  • Evidence of involvement in the development of programmes of care, protocols and audit.
  • Trained in PMVA techniques or willing to be trained.

Managerial/Supervisory
