Job summary

We are expanding our Intermediate Care Teams who deliver Discharge to Assess, Urgent Community Response and Virtual wards and are looking for Community Rehabilitation Assistants to join our supportive multidisciplinary teams across Dorset.

As the Community Rehabilitation Assistant, you will provide support to adult patients in their own homes, Community Hospital Wards or Care homes. You will promote independence and improve quality of life by supporting patients to remain at home whilst being supported by a multi-disciplinary team. You will work with a team of Nurses and Therapists to set goals and assist patients to regain their independence. You will provide admin support to help ensure the efficient day to day running of the service. Strong communication skills and confident decision-making are a must for this role. You will be joining a motivated, caring and compassionate team.

The Community Rehabilitation Assistant role is a full-time post, we would be flexible and welcome part time applications. Hours covered by the service are 08:00-20:00 7 days per week on a rota basis with weekend enhanced pay!Being a driver is essential for this role.

Main duties of the job

Providing rehab based care to people in their own home

Completing supervised assessments of mobility, equipment and function

Monitoring deteriorating patients with support from clinicians

Working with other Rehab Assistants to provide the service across 7 days
