We are looking for an enthusiastic, motivated, dynamic passionate Practitioner to Join the Pan Dorset Learning Disabilities Intensive Support Team.
This post is suitable for Preceptorship programme.
If you hold the following qualifications RNLD, RNM, experience of working with vulnerable adults / children who may have a learning disability, challenging behaviour, mental health diagnosis and co morbidities and would like to work in a specialist service this could be the role for you.
Main duties of the job
The team provides intensive specialist assessments to support people who may be experiencing crises in their lives, inform services for people who are in transition from child to adult services. Monitoring treatment and supporting proactive transition for clients who may be in out of area specialist hospital settings
Work in partnership with client, family, carers and other professionals and organisations involved in the person life, working alongside local Community Learning Disabilities teams, primary care, criminal justice, psychiatric liaison. working within client's home, care settings and third sector provisions.
