There is an exciting opportunity to join Dorset HealthCare's Community Mental Health well established team as a Community Mental Health Nurse/Practitioner.

As a Community Mental Health Nurse/Practitioner you will be required to have recent experience of working in a mental health environment with people suffering severe mental health whether this be inpatient, or community based.

In addition to this we are also happy to support you in your career by advancing your own skill set using additional learning opportunities such as NMP, ACP, and psychological therapies etc.

This is a full-time post, 37.5 hours per week, part-time hours considered.

For the role of Community Mental Health Nurse/Practitioner you will need to have a valid driving licence and use of a car, as you will be required to assess and monitor patients in their own homes and within other community settings.

Main duties of the job

The primary role responsibility is being a dedicated Care Coordinator for a caseload of patients with whom you'll provide person centred targeted support to assist in aiding their holistic recovery. You will be focusing on enabling service users to manage their severe and enduring mental health conditions and recover from them. You will provide high quality care and treatment in conjunction with other professionals through the Trust's agreed multi-disciplinary assessment and treatment/ care programmes. You will need to be able to apply the philosophies of engagement and recovery in your work with service users.
