Job summary

Are you a registered nurse or suitably qualified Allied Health Professional who is looking to enhance their career within our Sherborne Older Persons Community Mental Health Team?

This multi-disciplinary teamprovides recovery focused treatment plans for those people who are 65 and over with SMI or who have organic mental ill health including Alzheimer's disease.

The team is small, supportive and proactive both to service users and colleagues alike. We have a good record for retention of staff and are always looking for ways to improve our services and maintain high standards of care for those we serve.

We are an integrated team having good working relationships with Primary Care, Social Care and other partner organisations.

This is a challenging and rewarding role, but if you're ready for this then we are looking for someone who is reliable, committed and motivated with excellent communication, organisational skills and the ability to prioritise. Main duties of the job

To be personally responsible and professionally accountable for a patient caseload within the community, for people who are experiencing functional and/or organic mental health problems

To provide specialist care and interventions, assessments, treatments and managing patients on a caseload, as part of the wider multi- disciplinary care team.

To participate as an active member of the multi-disciplinary Community Mental Health Team (CMHT) in developing high quality and innovative community based services.

To work independently in the community, highlighting and communicating any known risks to the wider Multi-Disciplinary Team and taking appropriate action as required.

To provide specialist advice to others regarding the management and care of patients / service users.

Provide supervision, mentorship, training and clinical support to students and trained practitioners.
