Job summary

Come and join our Dorset Stroke and Neuro Service serving the West and North of Dorset. We have recently been awarded significant investment in Dorset to develop Stroke and Neuro Services across Dorset to meet the National Stroke Service model published by NHS England last year (particularly with regards to Integrated Community Stroke Services- ICSS). This is a really exciting time as we significantly expand our existing services and embed innovative ways of working to improve clinical outcomes for patients following stroke or neurological illness or injury. Importantly this once in a generation investment increases our team threefold and significantly extends the time patients can access the service moving to a needs led, rather than time limited service.

Our team expansion includes nursing, social work, neuropsychology and voluntary sector roles, as well as apprentices, rotations across the acute and community pathway and opportunities for advanced practice. Over coming years we aim to further bring together a range of specialists working across the fields of stroke and neurology in Dorset, meaning our integrated team will have access to a wide range of specialists with many years of experience, many of whom work within a national capacity within their field to increase opportunities for learning and development of skills as well as opportunities for involvement in research.

Main duties of the job

Our integrated community stroke and neuro team is AHP lead and will provide various pathways of care including supported self management, advice and signposting, intensive rehabilitation (ESD), complex management and reviews.

We have a programme initiated within Dorset developing communities of practice regarding psychological care and motor relearning, which post holders can join.

To provide a high quality person centred approach to care delivery which always considers people's safety, privacy and dignity.

To provide highly specialist care and interventions, assessments, treatments and managing patients on a caseload, as part of the wider multi-disciplinary care team.

To provide highly specialist advice to others regarding the management and care of patients / service users.

To devise highly specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.

To plan, implement and review health improvement programmes in a range of settings.

Provide supervision, mentorship, training and clinical support to students and trained practitioners.
