Job summary

West Dorset ICRT are looking for a Therapy Assistant Practitioner to work within the Multidisciplinary team, based at Bridport Community Hospital. The team consists of OT's, Physiotherapists, Nurses, Advanced Practitioners, Therapy Assistant Practitioners, Rehab Assistants and admin support who work as a team to provide intermediate care and rehabilitation to the adult population of West Dorset.

We are a friendly and dynamic team, that are committed to providing high quality care to the residents of West Dorset through integrated and collaborative ways of working in conjunction with our colleagues in Primary and Social Care sectors.

Main duties of the job

The post holder would be required to provide Holistic assessments and rehabilitation for a range of physical presentations such as stroke and neurology, orthopaedics, frailty, falls, cognitive impairment and many more. Skills required to fulfil this role include knowledge and experience in the community, skills to holistically assess and formulate a patient centred rehab program, knowledge and experience of working with the older person, particularly with frailty.

This role is a rotational post between the Community Hospital ward at Bridport Hospital and the Community Team, visiting patients in their own homes and Care Home settings. The successful candidate will be expected to demonstrate skills in communication, organisation, caseload management, autonomous working, and relevant competence and experience at the required level for the role.

EDUCATION, QUALIFICATIONS & TR

Essential

  • NVQ level 2/3 or equivalent award in an appropriate field
  • Willingness to complete further training for the role e.g. Apprenticeship

Desirable

  • Evidence of undertaking recent formal learning related to current role
  • Evidence of development within current role e.g. on the job training/ competency completion

EXPERIENCE

Essential

  • Experience in a care based workplace
  • Experience in care of the older person
  • Demonstrate enthusiasm for providing high standards of patient care
  • At least 2 years within Band 3 role
  • Experience completing autonomous basic assessments of a patient in relation to their function/ environment

Desirable

  • Working within the NHS
  • Able to recognise their own limitations
  • Experience setting and reviewing goals

SKILLS, ABILITIES & KNOWLEDGE

  • Basic computer skills for example entering numbers in a spreadsheet or using email
