We are looking for an enthusiastic, motivated, dynamic passionate Band 6 Registered Nurse Learning Disabilities or Registered Mental Health Nurse, or aBand 5 Learning Disabilities Or Mental Health Nurse post Registration 1 yearwho wishes to work towards a Band 6, to join a Multidisciplinary Specialist Learning Disabilities Intensive Support Team.

If you have experience of working with people with a learning disability, People with Mental health diagnosis and co morbidities and would like to work in a specialist service this may be the role for you.

Do you have an interest in working with people who may have a forensic history, experienced Trauma may have a Mental health diagnosis, or people who may struggle to maintain positive placements ?

The Team supports and encourages innovative practice, research based practice.

Personal development as a practitioner is fully supported through training both in house and externally and through other mediums such as shadowing.

The team's operational hours are 8am - 8pm seven days a week; shifts are on an allocated rota system and are 7.5 hours in duration .Working pattern is alternate weekends.

Enhanced payments for weekends and Bank Holidays are paid.

Main duties of the job

The team provides intensive specialist assessments to people with learning disabilities who may be in crises, working closely with specialist Children's services to support transitions from specialist child to adult services, support with the monitoring of people with learning disabilities placed in out of area hospitals and participate actively in planning their repatriation back into their local area or other identified placements. In reach in to local primary care and specialist services to support reasonable adjustments for people with a Learning Disability.

The team works in conjunction with the Community Learning Disabilities Teams, Criminal Justice Liaison and Diversion service and supports the in reach into Specialist / Primary Care Services.

We provide specialist therapy groups.

Specialist therapeutic approaches.

Training to carers and Care providers.

The Team are currently developing and piloting Video Positive Behaviour support plans and care plans these are co produced and form bespoke training packages for transitions and Care team development
