Job summary
An exciting opportunity has arisen for a trained nurse to join the community diabetes nurse specialist team within Dorset Healthcare NHS Foundation Trust. This post will allow the successful applicant to further specialise in the exciting, expanding and challenging field of diabetes. We are a small, friendly and supportive team, delivering up to date research-based care in a beautiful part of the country.
We are a nurse led unit but work closely with the other diabetes care providers in Dorset, including the acute hospital trusts and community nursing teams, community hospitals and residential care homes. We work to support primary care in delivering excellent diabetes care to people closer to home and reduce the need for secondary care referrals.
Main duties of the job
Working in the specialty of diabetes is challenging and rewarding and often requires innovative thinking. The successful applicant will eventually run joint clinics with a practice nurse where patients, and their diabetes treatment plans will be reviewed, evaluated and if necessary new treatment plans made. A large part of the role will be teaching both informally and more formally to patients and other health care professionals. Team work with the wider MDT team will also form a part of this role and creating links with other health care professionals will be important
The successful applicant will need to have some experience in the field of diabetes, be confident teaching groups of either patients or other health professionals, have excellent interpersonal skills and be innovative, enthusiastic and happy working autonomously. The role is in the community covering areas of North and West Dorset, therefore, being a car owner and driver is essential.
