The Older Persons Community Mental Health Team in Sherborne is looking for a full-time (37.5 hours) band 6 Community Mental Health Practitioner. This is an excellent opportunity to learn, develop and practice your mental health skills within a community setting.
We are a small but friendly multi-disciplinary team, based at The Yeatman Community Hospital, covering the Sherborne Primary Care Network.
We are committed to staff development and training is available to enable applicants who wish to enhance specific skills in support of this role.
Main duties of the job
The key areas of the role will be;
- Undertaking assessments
- One to one clinical interventions
- Caseload management
- Undertaking duty worker role
- Creating therapeutic relationships with people experiencing functional and/or organic mental health problems and their families and carers.
- Liaison with community, primary, other secondary and tertiary services and agencies within the locality.
