Job summary

Due to internal promotion an opportunity has arisen for a Band 5 Physiotherapist to join the multidisciplinary Purbeck Integrated Community Rehabilitation Team.

The post would suit a therapist with a commitment to integrated working with a passion for promoting rehabilitation, occupation and function as part of a multidisciplinary rehabilitation pathway. Training can be provided to develop any areas of specific clinical interest for both newly qualified and experienced band 5's. Professional support from experienced Band 6 therapists, access to the Trust's Preceptorship programme for newly qualified therapists, clinical supervision, and regular in-service training are just some of the benefits of working as part of this team. The other benefits involve working in a beautiful area of Dorset as part of a busy, friendly team, who are co-located with social services and community matrons which offers further scope for training and development.

Main duties of the job

The post will involve assessment and treatment of patients in both a community and inpatient setting (Purbeck locality and Swanage Community Hospitals) to prevent avoidable hospital admissions, promote earlier hospital discharges and rehabilitate a wide variety of conditions. The caseloads comprise of a mix of orthopaedic, neurology, respiratory and elderly care, so offer an opportunity to gain therapy skills across diverse patient groups.

The hours of work are flexible and would include being part of a rota at weekends and bank holidays once competency is achieved for this. We would consider applicants who are willing to work either part time (minimum 22.5 hours) or full time, 37.50 hours per week.Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

9. OTHER RESPONSIBILITIES

9.1 To work as part of a Multi-disciplinary team to provide effective Physiotherapy assessment and interventions patients in their own homes, care homes or, where applicable, Community Hospital wards.

9.2 The postholder will be expected to work autonomously, with supervision from senior team members, to assess, treatment plan and carry out interventions for a range of clinical presentations such as falls, balance, neurology, stroke, orthopaedics, respiratory and musculo-skeletal conditions.

9.3 The postholder will be expected to have sound knowledge in order to recognise the deteriorating patient and be able to escalate appropriately to senior colleagues or emergency services. This will include awareness of basic observations, pressure care management, medication, continence.

9.4 To undertake functional and holistic assessments that are evidence based and formulate patient centred goals to promote independence. To formulate treatment plans and delegate these to junior assistant staff where appropriate.

9.5 To evaluate treatment programmes and patients responses to treatments through the use of outcome measures, adapting goals and interventions where necessary.

9.6 Demonstrate good knowledge of standard equipment and autonomously prescribe and assess for this.

9.7 To manage own caseload ensuring prioritisation and timely input to patients that have been referred to the service.

9.8 To work as part of the wider Cluster Team to plan and facilitate discharges from Acute and Community Hospitals through the Discharge to Assess pathway.

9.9 To support the MDT to clinically triage incoming MDT referrals and allocate to a waiting list appropriately.

A sound knowledge of other Health and Social care roles is required.

9.10 To promote a self-management approach where appropriate; empowering service users to gain the skills and confidence for managing long term health condition wherever possible.

9.11 To assess for home adaptations to ensure the patients environment promotes independence and reduces the need for care

9.12 To demonstrate sound understanding of clinical governance by ensuring practice is evidence based and up to date. To actively seek learning opportunities through training, supervision and shadowing to achieve this.

9.13 To support Physiotherapy students under the supervision of a senior Physiotherapist.
