Job summary

We are seeking a proactive and highly motivated individual to join the Discharge Team at Frimley Park Hospital as a Discharge Care Coordinator.

The post-holder will play a key role in supporting safe, timely, and well-coordinated patient discharges, working both autonomously and as an effective member of the multidisciplinary team.

The successful candidate will be enthusiastic, well organised, and able to manage a varied workload in a fast-paced environment.

Excellent communication skills are essential, as the role requires regular interaction with patients, families, and health and social care professionals to ensure discharge plans are clear, appropriate, and person-centred. Strong IT skills are also required to maintain accurate documentation, update electronic systems, and support efficient information sharing.

Main duties of the job

  • Support effective discharge planning for patients with complex health and social care needs across all clinical specialties at Frimley Park Hospital.
  • Work closely with the multidisciplinary team to coordinate safe, timely, and appropriate discharge pathways, ensuring patients and families are fully informed and supported throughout the process.
  • Liaise with health and social care professionals to gather relevant information, identify barriers to discharge, and help develop person-centred plans that promote smooth transitions of care.
  • Use strong communication and problem-solving skills to manage competing priorities and respond to challenges in a fast-paced environment.
  • Work flexibly and independently while contributing positively to the wider discharge team, maintaining accurate documentation and upholding high standards of patient experience and service delivery.

  • As a member of the integrated Capacity and Discharge Team, you will work in partnership with social services, external stakeholders, patients, and carers to proactively support safe and timely discharge from hospital to home or onward care settings.
  • You will act as the single point of contact on a named ward, coordinating and contributing to discharge planning alongside multidisciplinary colleagues.
  • The role requires maintaining momentum throughout the discharge process by liaising closely with doctors, nurses, occupational therapists, physiotherapists, and Hospital Assessment and Discharge Team members, constructively challenging decisions when necessary to ensure effective planning.
  • You will screen all patients on admission to identify those requiring further social care assessment, completing referral documentation and updating relevant IT systems to activate referrals or re-referrals as appropriate.

  • Evidence of current CPD including evidence of recent study at least to NVQ Level 3.
  • Willingness and commitment to complete NVQ Level 4 in Health and Social Care within 2 years of appointment.

Desirable

  • Working knowledge and experience of EPIC
  • Knowledge of recent health and social care legislation and policy
  • NVQ Level 4 Health and Social Care.

Experience

Essential

  • Recent experience of working in a health or social care setting.
  • Experience of resolving conflict effectively.
  • Excellent knowledge of community-based resources.

Desirable

  • Experience of previous hospital or community working.

Skills & Knowledge

  • Knowledge of legislation and policy that has shaped current health and social care provision e.g. Gov.UK Hospital discharge service guide.
  • Evidence of good interpersonal skills with a wide range of people in a variety of situations and settings.
