Job summary
Cancer Pathway Trackers play a vital role in supporting the timely and effective management of cancer referrals. They are responsible for accurately documenting and monitoring patients' cancer pathways using the Somerset Cancer Register, ensuring that all relevant information is correctly interpreted and recorded in line with national cancer targets and Trust protocols.
This role requires excellent attention to detail, strong organisational skills, and the ability to work in a fast-paced, target-driven environment. Cancer Pathway Trackers are integral to maintaining high standards of cancer care and ensuring patients receive timely and appropriate treatment.
Main duties of the job
- Monitor and track patients on suspected cancer pathways using the Somerset Cancer Register and EPIC, ensuring accurate and timely data entry in line with national cancer waiting time standards.
- Review and interpret clinical correspondence (e.g. clinic letters) to identify and record the next steps in a patient's diagnostic or treatment pathway.
- Accurately transfer referral information from EPIC, e-RS, or referral letters into Somerset, maintaining high data quality and adherence to Trust protocols.
- Proactively identify, escalate, and document potential delays or breaches in the cancer pathway, following departmental escalation processes and liaising with the Cancer Services Team Leader and Cancer Performance Manager.
- Chase and upload clinical information such as imaging and reports from other hospitals into EPIC to support timely decision-making.
- Work both independently and collaboratively within the Cancer Services Team, prioritising workload effectively to meet strict deadlines and contribute to high-quality patient care.
