Previous ward-based experience is required for this role.

Main duties of the job

o To work within a multi-disciplinary team, in the delivery of high-quality patient care to patients & relatives. o Support shift leader ensuring that all documentation is up to date and report any changes in the patients' condition. o To contribute to the development and evaluation of nursing practice. o To demonstrate effective communication skills both written and verbal between patients and members of the multi-disciplinary team. o To communicate effectively with patients and their relatives. o To advise on the promotion of health & prevention of illness. o To liaise with appropriate clinical specialists in relation to individual patient needs e.g. Doctors, Occupational therapists, physiotherapists and In Reach matrons. To assist medical staff, ensure that treatments are effective and advise on nursing priorities. o To promote good working relationships with the ambulance service, police and other supporting agencies. o To act as an advocate for patients and their relatives/carers. o To communicate to colleague's individual need for support (through preceptorship programme)
