Job summary

The hospital at home / Virtual Frailty Ward / Urgent Community Response team provides an innovative, responsive, safe, effective and caring service that is well led.

The team is commissioned to provide urgent response at home (within 2 hours), to patients with frailty experiencing an acute illness or frailty crisis, to reduce or avoid hospital admission and promote patient choice in the delivery of care.

The hospital at home service, whilst based at Frimley Park Hospital, delivers a joined up approach across the local Frimley Health and Care Integrated Care System (ICS).

Main duties of the job

  • To work closely with the Consultant Geriatrician, Consultant Nurse for Older people, Advanced Nurse Practitioners and administrative team to provide rapid response services and professional, high quality patient care to patients & families in the community setting.
  • To participate in the development, implementation and evaluation of programmes of holistic integrated care whilst supporting carers between the hospital and home settings.
  • To assist the Hospital at home ANP's in managing the resources of the Team, including staff, in line with professional standards and quality of care.
  • To deputise for the ANP within scope of practice when required and to provide professional leadership within the team
  • To participate in the provision of acute medical care for frail adult people with long and short term nursing needs within the hospital at home service, on a rota basis, providing advice and care for patients requiring infusions, transfusions and other medications or procedures where it is deemed to be in the best interests of the person to be in a community setting and in agreement with the named nurse or consultant.
