Job summary

We have developed this role to work specifically with the OTs in our rapid response, admission avoidance and early discharge team. With a primary focus on rehab, you will be experienced in delivering rehab and treatment plans to patients, competent in adaptive equipment and able to motivate patients.

You will be lone working in patients' homes, undertaking delegated OT assessments, treatment, progression and reviews, as well as assessing for and, where appropriate, fitting equipment. You will have your own caseload and work autonomously, although always having ready access to both registered therapists and management.

We are a large team of OTs, Physios, Nurses, Rehab Support Workers, Associate Practitioners and Admin; we are extremely proud of what we do to support patients.

You will receive regular 1:1 clinical and personal supervision from a registered OT; we will support you with ongoing on-the-job training and professional development such as specialist courses: indeed we have an OT Apprentice in the team at the moment and would be happy to support the right candidate to consolidate their experience and achieve their registration.

You must be able to travel independently between visits in a timely manner, sometimes carrying equipment. You will be rostered between the hours of 8am-8pm 7 days a week.

Main duties of the job

To work with patients predominantly in their own homes providing assessment, rehabilitation, support and treatment programmes; undertaking home visits as appropriate. Working as a trusted assessor undertaking initial assessments and functional assessments as delegated by registered staff and supporting clinical staff with treatment and rehab input. To work within scope of competence.

To be proficient, knowledgeable and competent in rehab and functional assessment. Educated to Foundation Degree, Level 5 NVQ in Health and Social Care or be able to evidence that they are working towards one of these qualifications. To continue to develop in the role through the acquisition of further skills, knowledge and competences, to be determined by the needs of patients, the service and the team.

Job description

Job responsibilities

  • Within specific guidelines, and following training, to be solely responsible and accountable for a clinical case load working within codes of practice and professional guidelines. This includes patient documentation in both professional and medical notes.
  • Undertake occupational therapy assessments; often these will be the initial assessment on behalf of the Trust.
  • Gather information on the home environment of the patient and their premorbid and current level of functional ability.
  • Planning and implementing specific therapeutic programmes.
  • Once competent teach, supervise, and progress exercises for a specific caseload without direction.
  • This may include but not be limited to:
  • Mobility
  • Transfers
  • Soft tissue techniques
  • Balance and strengthening exercises.
  • Basic breathing exercises
  • Issuing and ordering equipment
  • Functional assessments including personal and domestic tasks
  • Motivating patients to exercise and address barriers to engagement.
  • To be responsible for performing given tasks and reporting any concerns to the registered therapist or appropriate member of the clinical team in a timely manner
  • To be aware of and adhere to Trust and local policies and procedures.
  • To provide programmes (exercise prescription) of activity that have a therapeutic basis following evidenced based guidelines laid down by the Specialist therapists.
  • To be aware of safe practise and the need for onward referral, particularly when carrying out exercise testing and exercise programmes.
  • To demonstrate a physical ability to carry out therapy interventions and therapeutic handling without direct supervision. There is a frequent requirement to exert moderate physical effort for several short periods during the day.
  • To be responsible for the safe use of therapy equipment, adhering to the Trust medical device policy. This includes competency and accuracy to use equipment by you, other healthcare professionals, carers and patients.
  • Liaising with the supervising registered therapist highlighting any variations to expected outcomes from agreed treatment programmes.
  • Developing person specific goals with individuals and their carers.
  • Monitoring the progress of patients and adjusting interventions where necessary.
  • To undertake risk assessments as per Trust policies and procedures.
  • To adhere to the Royal College of Occupational Therapist Code of Ethics and Professional Conduct.
  • Identifying the need for and initiating referrals to internal/external partners and agencies within agreed parameters.
  • Make patient referrals to other multidisciplinary teams/agencies. To receive and prioritize referrals to therapy made from internal and external agencies in discussion with registered therapist.
  • Lone working in community.
