Job summary

Provide an integrated model of care between primary and secondary care for cardiovascular services. This will increase accessibility to cardiology services locally which in turn will improve patient experience.

Our approach to this community cardiology model is to recruit and train a group of motivated and pro-active general practitioners who are committed to improving the standards of care for those patients with known or suspected cardiovascular disease in the community setting.

Main duties of the job

To deliver a community-based service for cardiovascular services

To improve the patient experience by providing care closer to home.

Provide expert advice in this specialist area and to enhance holistic patient assessment and treatment / management.

To strengthen primary care by enhancing interactions with secondary care which in turn will provide a wider variety of career opportunities for GPs.

About us

Frimley Health represents the merger of Frimley Park NHS Foundation Trust and Heatherwood and Wexham Park Hospitals NHS Foundation Trust, one of the largest Acute Trusts in the South East in 2014.

Job description

Job responsibilities

To be responsible for reviewing and assessing patients in a community setting.

To communicate the outcomes from the patient consultation and complete all necessary administration relating to the consultation including reviewing and requesting diagnostics and endorsing the results of investigations.

The GPwSI will make a diagnosis and provide recommendations to include lifestyle changes, medication, and request further investigations if requried. They will also be responsible to arrange any follow up required either in the community clinic or secondary care.

The GPwSI will be able to interpret results from the physiologist reports and communicate findings to the patient / GP.

The GPwSI will be responsible for triaging all GP cardiology referrals which can be triaged directly to the community cardiology service or to specific services within Frimley Health for example the heart failure service.
