The successful candidate will work closely with our critical care team and our new critical care rehabilitation MDT members providing holistic care on ICU and on ICU patient stepdown.

Our Critical care units at Wexham Park & Frimley park have an enthusiastic ,multi- disciplinary ethos and our aim is to provide early rehabilitation and therefore aim to minimise the impact of Post Intensive care Syndrome on our patients. You will work alongside our critical care team , lead nurse, physiotherapist ,psychologist and occupational therapist.

Main duties of the job

Within ICU

Have own ICU caseload, this will include; PROM, Bed/Chair/Standing exercises, Mobility, Thera trainer

Assist with doubles within physiotherapy caseload

Review all complex/comprehensive pathway patients:

o Physiotherapy: PROM, Bed/Chair/Standing exercises, rehab on edge of bed, transfer practice, mobility, Thera trainer (with or without registered physiotherapist as appropriate)

o Holistic management: delirium, orientation, suitable games to aid concentration, memory and brain exercises, emotional support, quality of life activities

o Occupational therapy as appropriate (advised by OT)

o Joint sessions with members of the MDT including Physiotherapy, Occupational Therapy, SALT, Psychologist

Support the MDT in ensuring patient diaries are completed and up to date

Support the MDT to complete objective assessments on ICU and record outcomes on the spreadsheet

Follow Up on the wards

Review patients that have been stepped down within 72 hours

Work with therapists/MDT to set patient centred goals

Joint sessions with Therapy teams on the wards to aid handover of patient

Additional treatments including; PROM, Bed/Chair/Standing exercises, Mobility, Thera trainer, functional rehabilitation

Providing patients with information about support following ICU and signposting as necessary

Objective measures as required and record of outcomes on spreadsheet

Liaising with Physiotherapist, Lead Nurse, OT and Psychologist
