Job summary
The discharge lounge offers a safe comfortable environment for patients whilst waiting to be discharged from hospital to their home. Patients are discharged through the Lounge to maintain and facilitate capacity and flow of the hospital, freeing up much needed beds for emergency and elective patients on a day to day basis. As a Staff Nurse working in the team you will work within the discharge team alongside site managers and ward team to ensure seamless discharges.
Main duties of the job
An exciting opportunity has arisen for a motivated and forward thinking staff nurse to join the team in the Discharge Lounge at Wexham Park Hospital.
Working with a well established team, the post holder will be able to work independently to work with the ward teams to move patients to the discharge lounge to support with capacity and patient flow.

- The prescribed medications required to take home on discharge. These will be explained by the trained member of staff.
- Follow up District nurse referrals if needed.
- Administer any medications as needed and indicated.
- Liaise Site Managers and Discharge Coordinators and care providers.
- Liaise with transport.
- Liaise with pharmacy.
- Contact next of kin.
- Check observations when needed.
- Health education on Discharge.
- Any support you in all matters relating to your discharge.
