Nurse Consultant/Lead Practitioner for Older People/Frailty Team

We have invested in growing our Senior Leadership team at the Community and Older Peoples Medicine Directorate here at Frimley Health and have an exciting opportunity for a dynamic Senior Nurse to join us as our new Nurse Consultant.
You will report to the Head of Nursing in the delivery of high quality patient care and our strategic ambitions within the Directorate.
The post is an ideal opportunity for an experienced Senior Nurse to build upon and advance their leadership, research, and managerial skills whilst being supported by the Divisional Head of Nursing, Frailty Team Manager, and Clinical Matrons. Ward Sisters, Clinical Senior Nurses, and Service Manager to deliver quality
Main duties of the job
Main duties
- In this exciting role, you will provide operational/strategic leadership across a broad range of Older Peoples teams at Frimley Park hospital, Emergency Department and the Hospital at Home Community teams.
- In both the Frailty team and Hospital at Home team, the post holder will act as the lead for safety, quality and patient experience, responsible for development and application of systems, control processes and risk management arrangements. To ensure full compliance with internal and external governance procedures and to benchmark against best practice requirements.
- You will lead the drive to transform our services, delivering continuous improvement projects across the Older Peoples Medicine Wards, leading process review, reviewing operational performance, and seeking effectiveness and efficiency through robust reviews audits and action plans.

- Advanced expert level of knowledge of management of older adults and individuals living with frailty and multiple co-morbidities
- Significant experience ( minimum 6 years ) working with older adults at an advanced clinical practice level, including experience within leadership and development roles
- Significant experience of facilitating change and service development to improve patient outcomes
- Demonstrated involvement with innovative practice developments and evidence of significant achievements gained
- Evidence of continued professional development and the application of evidence base to clinical practice
- Leadership and management skills including facilitation, supervision and assessment of others in the delivery of care
- Ability to lead and influence: within nursing, multi-professional working; within and across organisational boundaries
