Job summary

It's an exciting time for our Urgent Community Response (UCR) Service as we continue to build on our successful model. UCR services work with patients in their homes across Sussex communities to avoid hospital admissions and support those being discharged home.

Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. They deliver:

  • A rapid response to people in the community who are experiencing acute episodes of ill-health.
  • Acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

Main duties of the job

In this role, you will be the point of contact for service users, their families, and carers when information on their crisis and short-term care support is required. This will include enquiries about anticipated visits, providing updates, and listening to their concerns.

A key element of your duties will be processing referrals for reablement support and making onward referrals via the appropriate pathway for the patient, using criteria guidelines and seeking advice from relevant team members when required. You will sign-post these referrals to MDT members and other services as appropriate while monitoring, tracking, and reporting on progress within the patient pathway. This will involve escalating any issues or delays to the Operational Team Lead for appropriate action.
