Job summary
As a Registered Nurse, the post holder is required to adhere to NMC standards and guidelines of practice, education and conduct.
The role involves working as an autonomous expert Diabetes practitioner delivering evidence-based care and structured education to people with diabetes, their families and other health colleagues in a community setting. This includes participating in joint clinics & domiciliary visits for patients with diabetes including those living in Care Homes, who have complex needs whilst educating primary care staff to enhance their skills to provide an excellent diabetes service.
The Diabetes Specialist Nurse will be expected to teach and support programmes of care to individuals, primary care professionals and act as a clinical resource for community nurses.
The post holder will have specialist knowledge and skills in an area of Diabetes Specialist Nursing. The post holder will be expected to be a resource for other members of the team; support clinics across the locality as necessary and participate in clinical supervision.
The Diabetes Specialist Nurse will work with colleagues in primary and secondary care to develop an integrated care delivery system ensuring that this is based on research based practice.

Main duties of the job

Conduct home visits to housebound diabetes patients in homes, residential, or nursing settings, offering assistance and advice as requested by primary care. Use the SBAR process (Situation, Background, Assessment, Recommendations) for a holistic approach to diet, lifestyle, medication management, and insulin initiation, including end-of-life care.

Provide structured education for Type 2 diabetes patients to empower self-management, either in person or via Microsoft Teams.

Assist and lead educational sessions for healthcare professionals in primary care and community nursing teams.

Provide joint diabetes clinics (when competent) to support primary care, sharing knowledge to upskill healthcare providers.

Work autonomously to conduct patient reviews and offer clinical advice to both patients and healthcare workers.
