Job responsibilities

Job Overview

a) The post holder, working within West Sussex County Councils Family Safeguarding Model team, will act as a specialist emotional and wellbeing practitioner providing expert knowledge, skills and health interventions within the multidisciplinary (MDT) family safeguarding team. The post holder will work directly with both with adults and families, and indirectly through developing and delivering training and knowledge sharing with other health and social care professionals within the MDT.

b) Specialist emotional and wellbeing health practitioners are employed by SCFT and managed by the Family safeguarding Clinical Lead and work alongside a team of social workers and other social care professionals, working with parents where there is high risk of severe parenting problems including maltreatment and neglect.

c) As part of a multi-disciplinary approach, the post holder is responsible for providing high-quality emotional support to parents with children subject to child protection proceedings. This includes assessing, developing, implementing and evaluating interventions.

d) The post holder has the continuing responsibility for the management of a defined community caseload. There will be liaison with other agencies, supervision, deployment, and teaching of other staff and/or students.

2. Communication and working relationships

a) Assesses, plans, implements, and evaluates programmes of care for a designated client group.

b) Has a Lead role in a designated team, supporting continuing development and offering supervision, according to the SCFTs supervision strategy.

c) Works with others to deliver health and support services to ensure effective resource deployment and that the services are provided in a coordinated way.

d) Ensures families on the caseload are referred to appropriate support and advice services in the area.

e) Maintains and develops own clinical expertise through continued education and training and shares this expertise to ensure the delivery of a high quality evidence based service.

f) Attends relevant local meetings and forums, and networks with other professional teams and voluntary organisations within the area to ensure a comprehensive and coordinated service is offered to all families on the caseload.

g) Work alongside West Sussex County Council Childrens Services, attending group supervisions, child protection conferences and meetings and liaising with Social Care Practitioners to contribute to effective Family Safeguarding.

3. Main tasks

a) Provides, supervises, and supports the delivery of an effective programme of emotional wellbeing support, support and interventions to parents and adults in families (within competence) both within the organisation and with community partners.

b) Ensures effective liaison and communication with other professional groups within the Trust and outside agencies, acts as a health information source and works in partnership in the interests of clients.

c) Identifies health needs and themes across the Family Safeguarding Models caseload, in order to jointly develop and deliver appropriate services.

d) Takes a significant role, in partnership with others, in the development of public health initiatives for the Family Safeguarding Model service users, including the implementation, monitoring and evaluation of effective interventions.

e) Acts as a clinical expert in emotional health needs and safeguarding developing, leading, and participating in training, education and orientation programmes for staff, students and others as required.

f) Acts as a consultant for other non-health professionals within the Family Safeguarding Model MDT, providing information resource around public health, mental health and/or emotional wellbeing.

g) Incorporates relevant research findings into practice and participates in approved research programmes.

h) Identifies heath needs of families and adults within families on the caseload by undertaking an assessment in partnership with clients, in order to deliver an individual programme of care.

i) Maintains the safety and protection of children by following Child Protection Procedures, develops the service to promote the safety and wellbeing of children and participates in Safeguarding Children meetings, discussions, and case conferences as appropriate.

j) Keeps accurate, contemporaneous records and produces written reports as required.

k) Actively participates in regular clinical supervision in line with the SCFTs supervision strategy and in accordance with the WSCC Family Safeguarding Model.

l) Participates in analysis of the caseload to ensure an efficient and effective service and contributes to audit programmes as required.

m)Records relevant clinical/staffing issues/incidents and where necessary reports to the Service Manager in order to manage risk and maintain service delivery.

n) Maintains an accurate computerised data base of clients and inputs activity in order to support service planning and meet contractual requirements.

4. Main responsibilities

a) Actively supervises, guides, mentors, and supports the MDT team members to ensure that the delegated aspects of the care plan are delivered following evidence based practice and up to date guidelines.

b) Actively engages in the recruitment and selection of team members with support as appropriate from the Clinical Lead and manages the induction and orientation programmes for new staff.

c) Works with the Service Manager to ensure appropriate utilisation of allocated resources and works within budget restraints to deliver the Family Safeguarding Model service.
