We are looking for a dynamic, enthusiastic and motivated individual to join our workforce in a busy and well established community team.
This post is a fixed term contract for 18 months until February 2026.
Horsham PCN provides nursing services to an extensive area and has the privilege of working with a diverse population in a coastal town and the surrounding villages. Our community nurses benefit from having continual opportunities to develop an excellent skillset and become autonomous practitioners whilst working as part of a team delivering high quality, holistic care to patients in their own homes.
Primary Care Networks (PCNs) are an innovative service development involving community nursing. The ambition is to integrate multidisciplinary professionals so health and social care can be delivered seamlessly to those who need it. Due to this, community nursing offers a dynamic nursing environment which enables nurses to become confident, skilled practitioners.
Main duties of the job
a) Holistic assessment and care planning for patients in their own homes..b) Individualised treatment and the promotion of self-care. c) Admission avoidance and enabling patients to remain at home where possible
d) Cross service collaboration and referral to appropriate member of the multidisciplinary team.e) Development of extensive nursing skills with excellent training opportunitiesf) Professional development as required to maintain area of practice, asnegotiated in supervision.g) An awareness of health, safety and security risksh) Engagement in service provision and change.i) Acts as a practice supervisor for staff and students.
