Job summary

We are looking for a motivated and experienced professional with the relevant knowledge and commitment to provide excellent leadership skills and clinical expertise to the Referral Management Hub team based at Brighton General Hospital.

The role will be to provide senior leadership to staff working in the Referral Management hub, to facilitate and support the management of referrals, using clinical decision-making skills to Urgent Community Response Team and play a key role in management and leadership of staff. You will also support with the development of the service through reflective practice, clinical supervision, and audit.

Main duties of the job

The role involves liaising directly with GPs, and other community services, providing information and signposting to other departments and services if appropriate. Managing and coordinating a wide range of complex and competing priorities, supporting with safe and effective discharges you will be working closely with the acute trust, adult social care and other external partners, therefore excellent organizational and communication skills are essential for this role.

Essential

  • RN Level 1, or Allied Health Professional with HCPC Registration.
  • Evidence of training and development undertaken during current or recent employment.

Desirable

  • Mentorship Preparation Module or equivalent.

Experience

  • Minimum Two years' experience within the NHS or social care setting post qualification.
  • Experience of working in a multi-disciplinary team.
  • Experience of working in Community Setting.
  • Experience of using multiple methods to collect and record data to support service delivery.

Desirable

  • Evidence of Audit / Research in Practice.
