Job summary

The post holder will utilise research evidence based knowledge to act as an information resource for patients; multi-disciplinary colleagues and other stakeholders.

The post holder will hold responsibility for signposting and referring those individuals, families, carers and communities with whom they work to the most appropriate agencies/ services to support their needs.

The post holder will be responsible for designing, delivering and evaluating programmes of education, training and development on a one to one and group basis to MDT team members, and as required organisationally, systemwide and nationally, utilising professional presentation equipment and IT systems as appropriate.

The post holder will be expected to communicate effectively verbally and in writing with colleagues and stakeholders, utilising electronic methods of communication within the remit of the Data Protection Act ensuring that clinical information is conveyed within a timeframe that ensures patient safety.

The post holder will work collaboratively across primary and secondary care to establish and maintain communication to enhance patient experience.

Main duties of the job

  1. To work autonomously as an Advanced Clinical Practitioner, using advanced clinical skills to assess, investigate, diagnose, treat and manage patients. Such patients may have co-existing and diverse physical, psychological and social factors, making their management complex and multi-factorial.
  2. To develop, utilise and maintain skills beyond the scope of normal Nurse/AHP practice, e.g. advanced physical assessment, diagnostics and prescribing.
  3. To plan and direct the management of patients including accessing services within the multi-disciplinary team and wider health, social care, education, vocational and voluntary sector services as appropriate to role.
  4. To lead on the strategic development of trust-wide services in their specialist area, working with other strategic and clinical leads as appropriate.
  5. To lead and actively participate in education programmes designed to support other clinical staff in skills relating to the speciality.
  6. To provide peer support to clinical staff working with patients from the direct caseload or those involved in the management of patients demonstrating conditions relating to the speciality.
  7. To assist in the planning, co-ordinating and development of the service, including the evaluation of any newly implemented service plans.
  8. To be responsible for insuring that relevant legal and regulatory frameworks are acknowledged through local governance procedures.
