The post will be worked across both Therapy Hub and the 'in-reach' rehabilitation beds team Brighton and Hove.
This post involves working with patients with a wide variety of pathologies and medical conditions, who have recently been discharged from hospital, hospital avoidance and follow ups after input from Urgent Response Team, to help improve their mobility and independence to keep them safely at home or before discharge home from the in-reach bedded team.
In this role, your day will be varied and interesting, supporting patients towards achieving more independence in their activities of daily living. You will be supported by Physiotherapy, Occupational Therapy and Therapy Practitioner colleagues to review the needs of patients.
Your role will include patient-centered care in patients' own homes as per the care plans, focusing on rehabilitation and improving function and independence with activities of daily living. You will be able to work without the need for direct supervision of therapies staff but will have a supportive team around you. Implementation of treatment /care plans range from supervision of exercises and indoor or outdoor mobility practice, to washing, dressing and kitchen observations. You will undertake simple clinical and therapeutic procedures, such as taking physical observations and equipment provision. Efficient communication with all involved in the patient's care is key, as is clear documentation.

About us
We are looking for Therapy Assistants to join our exciting Therapy Service / in-reach bedded units covering x4 care homes in Brighton and Hove.
The successful applicants will support the multidisciplinary community team, aiding hospital discharges, prevent avoidable hospital admissions and provide rehabilitation in the community.
Due to the domiciliary nature of this post a full UK driving licence and access to a car is essential. The post is subject to successful interview, references, DBS clearance and Occupational Health checks.
