Job summary
The Enhanced health in care homes (EHCH) framework is for primary care networks (PCNs) and providers and commissioners of community health and social care services.
Sussex Community Foundation Trust have an important part to play in ensuring this framework is successful and alongside our established Care Home Matrons (CHM) an important and exciting element of EHCH is to introduce telemetry opportunities into the care homes both residential and nursing homes. We've completed Phase 1 focusing on Nursing Homes and to commence Phase 2 which will focus primarily on Residential Care Homes.
We've an exciting and interesting 1 year fixed term contract for a B4 Telemetry Care Home Co-Ordinator (TCC) who will be and autonomous person with a Digital/IT forward thinking outlook to support the Care Homes within East and West Sussex to support installation and functionality of the telemetry system via the Blue Box/Whzan initiative. The role requires significant travel and driving across the Sussex Counties as the role requires visiting the Care Homes who've nominated themselves for telemetry.
Main duties of the job
Visiting Care Homes within Sussex, who've nominated themselves to utilise telemetry, the Care Homes will be contacted by the Project Co-Ordinator (PCO) to arrange appointments for the TCC to visit and facilitate installation and set up of the telemetry box and its functionality.
The role is strictly non-clinical, and the TCC will escalate to the Lead Registered Nurse for for all issues brought to your attention, and understanding the limits of your role.
Keeping good, accurate and contemporaneous records at all times.
A patient and calm approach and an understanding of differing levels of Digital experience and knowledge within the individual Care Homes and temper your approach accordingly.
The TCC is a Monday - Friday role, but as a Flexible employer SCFT supports flexible working.
