Job summary

This is an exciting opportunity for an experienced diabetes specialist nurse with drive and ambition to have a clinical leadership and expert role.

The post holder will have clinical autonomy for assessing, evaluating and working with patients with complex Diabetes to formulate their care plan; identify opportunities for service development of community-based diabetes services including pathway implementation. This will be achieved through using clinical expertise to work alongside and inform the Nurse Consultant.

The post holder will provide advice and support of the highest level for all colleagues and patients with diabetes in order to maintain clinical excellence, reduce the number of emergency admissions, emergency bed days and to promote patient self-management and quality of life.

The post holder will provide clinical leadership and support to the Diabetes Specialist Nurses across West Sussex, as well as help with the training and up-skilling of practice nurses and allied health professionals in primary care.

Main duties of the job

The post holder will be expected to provide and lead on the delivery of high quality specialist integrated care for people with diabetes at all stages of the care pathway. You will act as a resource for other healthcare professionals and participate in nurse led clinics across West Sussex.

Job description

Job responsibilities

  1. Work autonomously providing clinical guidance and advice.
  2. To provide contemporaneous records and reports regarding patients ongoing care and management consistent with legislation, policies and procedures
  3. To assist with supervision, recruitment, induction and appraisal of junior staff
  4. To facilitate education programmes tailored to the needs of patients with diabetes, underpinned best practice and using a patient centered approach
  5. To support the efficient delivery of the diabetes service across West Sussex
  6. To assist in the provision of the service in accordance with the wishes of the commissioners.
  7. Delegate work appropriately to other clinical staff
  8. To support personal development planning and objective setting and review. To assist with performance management issues as required.
  9. To maintain competence in specialist area of practice by means of peer review, clinical supervision, journal and other reading and attendance on courses and conferences.
  10. Prescribe and review medication (if appropriate prescribing qualification has been obtained and governance in place) for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national practice protocols, and within scope of practice.
  11. To participate in team meetings and update staff on operational matters, policy updates and training requirements.
  12. Review departmental and corporate policies to update in line with new legislation and actively assist in development of policies that directly involve diabetes.
  13. To assist participation of department in audit in collaboration with the departmental audit lead.
  14. To assist professional manager and patient safety lead in matters pertaining to patient safety and risk management.
