SCFT delivers excellent care at the heart of the community for people living in West Sussex. OneCall Coastal is looking to appoint a motivated and enthusiastic individual to carry out the role of Senior Nurse Assessor. This post is open to all suitably qualified nurses.

Main duties of the job

The main focus for OneCall Coastal is hospital avoidance when appropriate. This is achieved through the telephone triaging of patient needs and accessing or signposting to the appropriate community services wherever possible, or alternatively facilitating hospital admission to either the community or acute hospitals. In addition to this, OneCall also provide support to ECHO (End of Life Care Hub), which is also based in the OneCall office. The post holder will need to have the clinical knowledge and capability to assess and refer the patient onwards to the appropriate service. Good communication skills are essential. The service receives over 400 calls per day. We aim to answer all calls within one minute with the referral details accurately recorded onto the SystmOne database. We also receive referrals via email.The successful candidate will work alongside a team of Senior Nurse Assessors, Nurse Assessors and Coordinators.
