An exciting oppurtunity has arisen for an experienced Band 6 Nurse or AHP to join the award winning and NICE recognised SCFT Carers Health Team!

The Carers Health Team is an innovative service that aims to improve health outcomes for carers. The service provides professional healthcare input for informal (unpaid) carers over the age of 18 across West Sussex. The role involves lone working, home visiting, carer clinics, health promotion and outreach work (very occasionally this may be either at the weekend or in the evening) including giving presentations and attending multidisciplinary meetings.

This role involves lone working, caseload management and integrated partnership working with the wider SCFT community services, social services and the voluntary sector.

Whilst the service covers the whole of West Sussex this post will predominantly be based in the North of the county covering areas such as Horsham/Crawley/East Grinstead/Mid-Sussex but with a willingness to visit patients in the coastal region if required, and attend meetings across the county.

Main duties of the job

The purpose of this service is to support carers through health and wellbeing interventions so that the carer and cared-for person achieve the best possible quality of life; relief from symptoms and flexible choices about their management. This is an anticipatory, health promoting model of care for adult carers and their cared-for and includes the identification of new carers.

In order to meet the stated outcomes you will be undertaking:

  • An holistic assessment of health, social and environmental needs of informal carers living in the community and carry a caseload of carers, and sometimes their cared-for, to enable proactive choices and advanced care planning with regard to the management of their condition and caring situation.
  • Close liaison and working partnerships with MDTs in the health, social and voluntary sector
  • Service promotion with attendance at community events and MDT meetings

About us

You will be expected to work closely with Community Nursing Teams; Proactive Care Multi-Disciplinary Teams; G.P.'s & Primary Care Teams and other health and social care professionals along with other charitable and voluntary services in order to ensure a seamless and equitable service for carers.

You will need to have a good understanding of the role of the informal carer and be someone who can enable Carers to understand the importance of their own health and wellbeing needs as well as those of the person they are caring for.

Applicants will need to demonstrate a willingness to work flexibly and co-operatively with colleagues within the Carers Health Team and wider multidisciplinary teams across the whole SCFT area.The applicant must be a driver and car owner.As the team work over the whole of the SCFT area, staff must be willing to travel within this area.
