Job summary
An exciting opportunity has arisen to join the High Weald Lewes and Havens Community Neurological Rehabilitation Service ( HWLH CNRS ) as a Band 3 Rehab Support Worker ( RSW ) .
The Team specialises in rehabilitation of patients who have had a Stroke or Brain Injury and we predominantly work in the patients place of residence.You will supported by the MDT who consist of Physiotherapists, Occupational Therapists , Speech Therapists, Nurses, Neuropsychologist and other RSW's.There will be lots of opportunities for development, and you will be supported to meet key core competencies and to develop your skills in this field.
Currently our service operates Mon-Fri, 8.00 - 5.00, but there may be a requirement for seven-day working as we move forwards.
Being a car driver is essential for this role.
Full and Part time hours are available.
Main duties of the job
In this role, your day will be varied and interesting, supporting patients towards improving function and achieving more independence in their activities of daily living. You will be supported by Therapists to review the needs of patients.
Your role will include patient-centred care in patients' own homes as per their goals and therapy programmes. You will be able to work without the need for direct supervision of therapies staff but will have a supportive team around you.
Implementation of treatment plans range from supervision of exercises and indoor or outdoor mobility practice, to washing, dressing and kitchen observations. You will undertake simple clinical and therapeutic procedures, such as taking physical observations.
Efficient communication with all involved in the patient's care is key, as is clear documentation.
You will be part of a supported induction programme and offered job-relevant training to ensure that you feel confident and able to undertake this role to the best of your ability, whilst being able to reach your own potential through personal development along with peer and supervisory support.
