Job summary
It's an exciting time for our Urgent Community Response (UCR) Service as we continue to build on our successful model. UCR services work with patients in their homes across West Sussex communities to avoid hospital admissions and support those being discharged home.
Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. They deliver:
- A rapid response to people in the community who are experiencing acute episodes of ill-health.
- A supported discharge service to facilitate timely hospital discharges.
- Acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.
We have an exciting opportunity for a senior Nurse to join us in a new role for our UCR teams, as a Community Liaison Clinical Lead (Nurse). Based in our local acute hospitals, you will work with teams there to support admission avoidance for people by ensuring the appropriate urgent community response is in place to support them to remain at home where appropriate to do so. Main duties of the job
The role is based within the acute setting and will work to identify and support mainly admission avoidance patients who require community services to prevent admission and signpost for the most appropriate service. The roles focuses on providing safe, effective, and quality senior clinical care, along with strong clinical leadership, advice, and support to the team.
You will use your comprehensive knowledge and champion excellence in practice in a dynamic healthcare environment. Your specialist clinical assessment skills will support patients with complex needs in addition to providing clinical advice and support to colleagues.
Working with the operational leads both in the acute and community settings, you will ensure the clinical effectiveness of services and the delivery of an excellent patient experience. You will also be part of the development of the service through reflective practice, clinical supervision, and quality improvement initiatives. Central to this is supporting the principles of interdisciplinary working and cross skilling for all staff supporting each other and learning together.
Your experience of working across a variety of clinical settings and working autonomously will help you excel in this role. We will support you to develop specialist areas of practice and your leadership skills through internal and external programmes.
