Job summary

Community Sister/Charge Nurse - 37.5 hours per week

We are delighted to offer an exciting opportunity for the right person to join our Adur Team (Lancing and Shoreham), as a Community Sister/ Charge Nurse.

We work as a multi-disciplinary team which includes GP's, Therapists, Specialist services, Social Care, Mental Health and voluntary services.

The role includes coordinating and delegating individual patient care by working across boundaries within secondary and social care settings. The role is that of a caseload holder, providing clinical case management for patients with complex health. Assessment and planning of needs, liaising with other professionals. To promote quality of life with support, encourage self management where appropriate, be proactive to prevent unnecessary/ avoidable hospital admissions, or an unnecessary length of hospital stay.

The successful applicant can expect to have an individually planned and supported induction to post with ongoing development covering all the clinical skills required for the role.

Main duties of the job

  1. a) In conjunction with other key professionals you will be accountable for a patient caseload and delegate responsibility to other professionals within the multidisciplinary team.
  2. b) To apply advanced assessment of physical, mental and social care needs to provide clinical care and health promoting interventions that help avoid unnecessary hospital admission.
  3. c) To interpret clinical signs and symptoms and diagnostic tests in order to develop a treatment plan and /or treatment in conjunction with medical practitioners as appropriate. Provide advanced clinical advice to the wider healthcare team to improve outcomes of patient care.
  4. d) Enable patients to be as independent as possible facilitating a range of self management strategies.
  5. e) Ability to develop and analyse audit, interpret data and use findings to enhance clinical practice.
  6. f) Supervise and support junior staff and students in their learning and development 7) In conjunction with other key professionals you will be accountable for a patient caseload and delegate responsibility to other professionals within the multidisciplinary team.
