Job summary

Diabetes Care for You is a multidisciplinary community diabetes service covering Brighton, Hove, High Weald and the Havens in East Sussex. The service comprises of consultants, nurses, dietitians, podiatrists, psychologists and support staff, that all contribute to the development of this innovative service

Main duties of the job

We are seeking to recruit a Senior Diabetes Specialist Podiatrist to work within our Podiatry team.

The Senior Diabetes Specialist Podiatrist will work in partnership with patients with type 1 or type 2 diabetes with moderate, high risk and active foot problems in a community setting, aiming to improve outcomes for patients. This includes the development of clinic-based care, home visits, along with healthcare professional and patient education programmes for diabetes management.

The post holder will also establish and maintain effective working relationships with health professionals in both primary and secondary care. They will work alongside the Lead Podiatrist to develop their clinical skills, and those of the team of Band 5 and Band 6 Podiatrists, to ensure the provision of high quality specialist podiatry care to patients. This role offers an excellent opportunity to work within a multidisciplinary team integrating physical and psychological support.

To ensure the provision of a robust, integrated arrangement of care for diabetic feet within thecommunity diabetes service and across the interface between Tier 3 and Tier 4 patients, providing a consistent high quality and a systematic approach to patient care.

The post holder will carry a high risk diabetic clinical caseload, using their highly developedspecialist skills to ensure that patients receive the best evidence based care package.The clinician will be required to participate in regular Multi-Disciplinary Team Meetings withinthe service in order to input into this forum of patient careThis role will require the triage of incoming referrals ensuring that they are allocated to the rightlevel of speciality within the service, or referring on to other specialities, services or agencies asappropriate.To clinically supervise Band 5 and 6 members of the podiatry team, acting as a clinical mentor,this role extends to students placed within the service and clinicians on rotation from the podiatry service. The post holder will be accountable, with the Lead Podiatrist, for the delivery of safeeffective Podiatric care within the pathway.To participate in team meetings and to regularly update the staff on developmentswithin the field of diabetes.To attend regional and local expert reference groups.To attend and input into Sussex Community NHS Trust podiatry service Continuing Professional Development events and facilitate Band 5 & 6 team members attending.To participate in patient satisfaction and audit studies as deemed necessary by the servicemanager and Lead Podiatrist.To lead the education sessions with other providers in matters relating to diabetic podiatriccare.Prescribe and review medication (if appropriate prescribing qualification has been obtained andgovernance in place) for therapeutic effectiveness, appropriate to patient needs and inaccordance with evidence-based practice and national practice protocols, and within scope ofpractice.

  • BSc Podiatry or equivalent
  • MSc (working towards) or equivalent experience
  • Evidence of Post graduate Continual Professional Development
  • Evidence of working at nationally accepted competency level for Band 7 Diabetes Specialist

  • At least 5 years post graduate experience preferably covering both acute and community clinical work. At least 3 years of this must involve specialist care for the patients with diabetes

  • Comprehensive evidence of relevant lifelong learning and post graduate training
