Community Staff Nurse - full and part time positions available
An exciting opportunity has arisen for a Community Staff Nurse to play a key role in providing a range of nursing skills to patients with a variety of complex and non-complex needs, in order to both proactively and responsively prevent hospital admission.
Main duties of the job
Primary Care Network (PCN) Community teams are multidisciplinary teams encompassing both Community Nursing and Proactive Care. They are the core element of a wider system responsible for coordinating care delivery to patients on the caseload. The delivery of high quality care is organised around an individual's needs, identified through holistic assessment and referral, and centered around collaborative working by a range of service providers.
The post holder will be working without direct supervision in a number of community settings, following the nursing process and delivering appropriate nursing care to patients on the caseload. They will also be an information resource for patients and colleagues, and work collaboratively with our health and social care partners.
Applicants should be nurses with a wide range of clinical competencies including but not limited to, wound care, catheter management, palliative care, phlebotomy, administration of medications/injections, and compression bandaging. Development opportunities will be available for the right candidate once in post if further competencies are required.
