Job summary
This is a rare and exciting opportunity to join the Primary Care Network (PCN) Community Teams in north West Sussex, providing integrated working among local organisations meeting the holistic needs of patients in the community to prevent hospital admission or residential care placement. As the Psychological Therapist you'll be a key member of the team ensuring that the emotional needs of patients are understood and met. Alongside this you'll also support and develop colleagues in promoting a holistic, person-centred approach, embedding psychological thinking and formulations within the team.
This is an innovative approach to healthcare so we're looking for someone with enthusiasm and creativity to work with multidisciplinary colleagues across different organisations. Working across 2 MDTs in Crawley and East Grinstead, you'll hold a small caseload of direct clinical work, delivering brief, time-limited psychological interventions where needed. You'll also be expected to contribute to case discussions, multidisciplinary assessments and contingency plans.
You'll mostly see patients for face-to-face treatment sessions in their homes, ensuring social distancing is respected, so you'll need to be mobile across the locality. We also offer video sessions and IT equipment will be provided. Based at Crawley Hospital there is opportunity for you to work from home for some of the week.
Main duties of the job
Being highly motivated, you'll need to be good at organising your diary and liaising with various professionals within and external to the Trust. Promoting and providing reflective practice sessions to the teams is a key part of the role for which you'll need to be a good team player, friendly with excellent communication skills. Being one of three Therapists embedded within PCN Community Teams means you'll have access to support and supervision on a regular basis, alongside CPD opportunities throughout the year.
To be responsible for organising the PCN Community Team's psychological therapy provision including reporting systems, documentation and record keeping, compiling reports as required according to Trust and service protocols.
Promote and develop MH awareness and psychological mindedness through contact with the general public and other clinical staff, and through training and events / conferences.
To participate in the development of the service to meet patient and primary care needs through data collection, clinical audit and service evaluation with reference to published research, NICE and NSF mental health directives.
To participate in service education activities as required, and in collaboration with Clinical Services Manager and Clinical Supervisor, identify on-going professional needs for continuing professional development.
