Job summary

A new opportunity has arisen for a Band 6 Senior Nurse to join our Responsive Services Team based in Haywards Heath.

We are a friendly multi-disciplinary team providing nursing support across Admission Avoidance and Supported Discharge pathways for the local population.

We have recently implemented a 2- hour Urgent Community Response model to further prevent admissions, supporting patients to continue living independently in their own homes.

Main duties of the job

The service operates 0800-2000 with flexible shift patterns spread across 7 days. You need to be a car driver and have access to your own vehicle with business insurance.

We would like an autonomous, enthusiastic nurse who can use enhanced clinical reasoning and apply a wide range of practical nursing skills. Strong candidates will demonstrate excellent leadership skills, the ability to react according to priority and remain calm under pressure.

  • To assess patients with other members of the Responsive Services (RS) team to prevent admission to hospital and support the discharge from hospital.
  • To provide nursing input for patients already on the caseload. Including medication management, wound care, catheter care and care of the deteriorating patient.
  • Completion of further assessments such as respect process, CHC funding applications, and continence assessments
  • To have a responsibility for ensuring that the MDT of Nurses, Therapists and Healthcare assistants work effectively in providing and facilitating a high standard of rehabilitative support for patients receiving RS care.
