Job summary
The clinical nurse specialist will act as a key worker for patients throughout their cancer journey.
Provided providing person centered compassionate care to patients and their families.
Holistically assess patients' needs and plan individual person centered are around those needs.
Have an in-depth knowledge and expertise in the specialty to provide patient information, advice and clinical care. To provide education and be a resource to other health care professionals.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
- Acts as advocate for patients and their families
- Delivers evidence based individualised and personised care through holistic needs assessment, planning and evaluation of care and all care interventions
- Assesses patients information needs and provides relevant information to meet those needs
- Enables patients to access, identify, appraise and interpret information, to determine their own future
- Provides clinical expertise, specialist advice and support across organisational boundaries
- Develops clear pathways of care, referring to other professionals and signposting to other agencies as appropriate
- Promotes the service and clinical excellence by publishing and presenting innovations, audit and research both locally and nationally
- Undertake quality improvement initiatives relevant to role and service
- Actively participates in multi-professional meetings, acting as patient advocate and representing nursing views
- Undertake assessment and management of clinical symptoms related to the disease process and or treatment related toxicities
